Knee replacement recovery: week 1
Clinical procedure: Total knee replacement.
Typical, evidence-anchored estimate only. It is not medical advice, not a prediction for your body, and not a substitute for your surgeon or clinician.
what's typical at week 1
typical pain · week 1
Around week 1, the typical pain band sits at 4.7/10 (range 3.5–6.5). This week was measured directly in the cited cohort.
weeks 0–1
Weeks 0-1 · acute and supervised
Hospital discharge happens in one to three days for most patients; some centers now do same-day discharge for selected cases. VAS pain on day one averages 6/10 (range 4-8). The PMC9741301 cohort measured a median NRS of 4.7/10 at the day 8-9 transition — slightly down from acute, with the residual driven by motion work rather than rest pain.
Most people are walking with a walker or crutches from day one. Mobilization within 24 hours of surgery is now standard. Compression stockings, ice, and elevation form the core of the home regimen.
Range of motion goals at the end of week one are typically 70-80 degrees of flexion and full extension. Loss of extension is the single mistake worth avoiding — chasing flexion past 90 degrees is much less important early than maintaining the ability to fully straighten the leg.

Home within days
Most people leave hospital 1 to 3 days after the operation, walking with a frame or crutches from day one.
Good to know
The day 8-9 bump is the rehab transition
Daily tracking shows pain reaching its early low on day 8 (4.6/10), then ticking up on day 9 (4.8) as rehab intensity shifts — before resuming the fall to 3.0 by day 29. A bump here is in the data.
What's typical
Nearly everyone hurts right now
94% report pain on the day of surgery, averaging 5.8/10. If this week is rough, you are in the overwhelming majority — the curve bends from here.
The pattern
The knee can stay warm to the touch for up to a year
Across pooled studies, the operated knee runs warmer than the other knee, biggest in the first 2 weeks at about 2.8C hotter, then 1.4C at 3 months, 0.9C at 6 months and 0.6C at 12 months. It was still measurably warmer than the non-operated knee even at one year.
The pattern
Swelling drops fast then lingers for months
Measured by bioimpedance, knee swelling jumped about 35 percent above pre-surgery levels on the first day after surgery, then came down but was still about 11 percent above pre-surgery levels at day 90. Swelling that hangs around at three months tracked with weaker quadriceps and slower walking in this group.
The pattern
Night pain usually fades after about six weeks
In one cohort, nocturnal knee pain was reported by 38.7 percent of patients before surgery and by 2.9 percent at one year. Night pain eased faster after a partial replacement than a full one, with the turning point landing around six weeks after surgery.
The pattern
Poor sleep can persist even after the pain settles
In the same cohort, poor sleep quality was reported by 54 percent of patients before surgery and was still 53 percent at one year, even though night pain had fallen to about 3 percent. Sleep and night pain did not move in lockstep here.
Coming up
About 1 in 60 knees gets a manipulation for stiffness
In a nationwide registry of 64,840 knee replacements, 1.7 percent had a manipulation under anesthesia within the first year because the knee was not bending far enough on its own. So this extra procedure for stiffness is something a small minority of knees go through, not the typical path.
Coming up
When the stiffness procedure happens, most are early
Among knee replacements that needed a manipulation under anesthesia for stiffness, 60 percent of those procedures were done within the first 3 months after surgery. So if a stiff knee is going to get this extra step, it tends to cluster in the early window rather than spreading out across the first year.
Coming up
Bending past 110 degrees by week two
In a cohort of 273 knee replacements, 57.5 percent had reached more than 110 degrees of bend with near-full straightening by the 14th day after surgery. That leaves a large share who had not gotten there yet at two weeks, and the study notes this early reading does not lock in the final result.
