Almost everyone expects to kneel again, far fewer end up doing it

In the same body of research, 94 percent of people expect to be able to kneel a year after a knee replacement, while only around 37 percent actually report kneeling at that point. That gap between what people anticipate and what tends to happen is one of the wider ones in knee-replacement recovery.

The knee can stay warm to the touch for up to a year

Across pooled studies, the operated knee runs warmer than the other knee, biggest in the first 2 weeks at about 2.8C hotter, then 1.4C at 3 months, 0.9C at 6 months and 0.6C at 12 months. It was still measurably warmer than the non-operated knee even at one year.

Swelling drops fast then lingers for months

Measured by bioimpedance, knee swelling jumped about 35 percent above pre-surgery levels on the first day after surgery, then came down but was still about 11 percent above pre-surgery levels at day 90. Swelling that hangs around at three months tracked with weaker quadriceps and slower walking in this group.

How fast the thigh bounces back to its own baseline

In a cohort tracked before and after surgery, the operated knee's extensor muscles measured about 42 percent weaker than their own pre-surgery level at 1 month. By 3 and 6 months those same muscles had returned to roughly their pre-surgery strength.

In one cohort, nocturnal knee pain was reported by 38.7 percent of patients before surgery and by 2.9 percent at one year. Night pain eased faster after a partial replacement than a full one, with the turning point landing around six weeks after surgery.

In the same cohort, poor sleep quality was reported by 54 percent of patients before surgery and was still 53 percent at one year, even though night pain had fallen to about 3 percent. Sleep and night pain did not move in lockstep here.

Across a year of follow-up, average insomnia severity rose from before surgery to a high point at six weeks, then fell below the starting level by three months and kept easing through the first year. The roughest sleep window tended to sit in the first weeks, not at the finish.

Across pooled data, about 34.5 percent of people could kneel within the first year, rising to 36.8 percent by at least a year out and 47.6 percent by three years or more. Kneeling is one of the slowest knee-replacement abilities to return, and for many it stays partial well past the point other movement feels normal.

Straightening barely moved, sitting near 3 degrees short of full at two weeks and 1 degree short at a year.
