Weeks 6-12 · strength phase
Open-chain quadriceps work starts at six weeks per the MOON guideline — the historical concern about graft strain has been largely retired by recent evidence. This is where measurable strength returns. By twelve weeks most BPTB and quadriceps grafts are running on a pool or unweighted treadmill; the University of Virginia protocol uses week 7-8 for pool running and reserves full-weight-bearing running for week 12 (BPTB) or week 16 (hamstring).
Return-to-work clusters in this phase. Ozbek's systematic review of 1,791 patients reported 82.7% back to work by 75 days and 90.2% by 90 days. The graft split is real: quadriceps autograft averaged 46 days, allograft 70 days, LARS 49 days, hamstring 84 days. So plan around 7 to 11 weeks depending on graft and job type.
Pain in this window is interpolated and typically reported as 0.5-1.5/10. If the knee is still swelling visibly after activity at week 12, that is worth raising — persistent post-activity effusion is a well-recognized red flag for graft or meniscus issues and is not a normal feature of the strength phase.

Out of the pool and onto land
Around this stage protocols progress running from the pool or an unweighted treadmill toward land running, once quad control and gait look right. It is gated on how the knee moves and recovers, not the date, so easing into it over a few weeks is the normal shape.

When extension loss does become symptomatic it tends to build over about 4 months
If a cyclops lesion turns symptomatic, the roughly 20-degree limit on straightening it can cause is described as building over about 4 months after surgery rather than all at once. On MRI these lesions are common and mostly silent: incidence rose from about 25% at 6 months to 33% by 1 to 2 years, and a separate series found 46.8% at 1 year of which only 10.6% were symptomatic.
The pattern
Kneeling pain tends to be the sharpest early, then eases
In a group followed after a patellar-tendon graft, kneeling produced more anterior knee pain than any other movement tested, while stair climbing, running, jumping, squatting and sitting cross-legged did not provoke moderate or severe pain. The share with considerable kneeling pain fell from 48.5% at three months to 25.7% at six months.

Where it hurts after a patellar-tendon graft, and how often
After ACL reconstruction with a bone-patellar tendon-bone graft, the front of the knee is a documented site of lingering soreness. Across Level I trials, anterior knee pain was reported by anywhere from 5.4% to 48.4% of people, and pain specifically with kneeling by 4.0% to 75.6%. The wide ranges reflect how differently studies measured it, not a single fixed number.
The bigger picture
Patellar-tendon vs hamstring graft and the chance of a second surgery
Across 45,998 ACL reconstructions in the Scandinavian registries, the risk of needing a revision was lower with patellar-tendon grafts than with hamstring grafts (hazard rate ratio 0.63). The same direction held across age, sex, and cartilage-injury subgroups. Which graft you have is decided in the operating room, so this is a difference between recovery paths rather than something a person changes after surgery.

Needing a procedure to free up a stiff knee is uncommon
In an insurance-database review of 7,798 people who had ACL reconstruction between 2007 and 2017, 115 of them — about 1.5% — went on to need a manipulation under anesthesia within the first year to address stiffness. The other 98.5% did not.
