Strength gates open in this window. The Mass General Brigham protocol uses an 80%-of-the-other-leg gate around weeks 8-12, by which point most people are doing single-leg work and full range squat to chair height.

Six-month VAS in the PMC10399043 cohort had a typical value of 1.5/10, but a high band of 4.1/10 reflecting the residual-pain subgroup. So the average is encouraging and the spread is wide. If your pain at six months is materially above 3-4/10 in daily life, that warrants a follow-up — not because anything is necessarily wrong, but because the pathways for unexplained TKA pain are well-mapped and treatable.

A stiff-knee setback, and what the manipulation recovers

In a 25-year single-center series of 12,735 knees, 3.6 percent developed lingering stiffness and 2.6 percent had a manipulation under anesthesia for it. When a manipulation was done it added about 34 degrees of bend on average, though those knees still ended a little stiffer than knees that never stiffened, 102 versus 116 degrees. It is a documented bump that often partly, not fully, resolves.

Almost everyone expects to kneel again, far fewer end up doing it

In the same body of research, 94 percent of people expect to be able to kneel a year after a knee replacement, while only around 37 percent actually report kneeling at that point. That gap between what people anticipate and what tends to happen is one of the wider ones in knee-replacement recovery.

When lingering pain feels like burning or tingling

Among people who still had troublesome knee pain three months after surgery, about 53 in 100 screened positive for nerve-type pain on one questionnaire and about 74 in 100 on another. Pain that feels like burning, tingling, or electric shocks is a recognized and documented pattern in this group, not something unusual.

The knee can stay warm to the touch for up to a year

Across pooled studies, the operated knee runs warmer than the other knee, biggest in the first 2 weeks at about 2.8C hotter, then 1.4C at 3 months, 0.9C at 6 months and 0.6C at 12 months. It was still measurably warmer than the non-operated knee even at one year.

Swelling drops fast then lingers for months

Measured by bioimpedance, knee swelling jumped about 35 percent above pre-surgery levels on the first day after surgery, then came down but was still about 11 percent above pre-surgery levels at day 90. Swelling that hangs around at three months tracked with weaker quadriceps and slower walking in this group.

The strength gap between your two legs, in numbers

Pooling five studies, the operated knee's quadriceps measured weaker than the other leg by about 27 Nm at 3 months, narrowing to about 18 Nm at 6 months and about 16 Nm at 1 year. The gap shrinks fastest early on, then keeps closing more slowly through the first year.

How fast the thigh bounces back to its own baseline

In a cohort tracked before and after surgery, the operated knee's extensor muscles measured about 42 percent weaker than their own pre-surgery level at 1 month. By 3 and 6 months those same muscles had returned to roughly their pre-surgery strength.

Night pain usually fades after about six weeks

In one cohort, nocturnal knee pain was reported by 38.7 percent of patients before surgery and by 2.9 percent at one year. Night pain eased faster after a partial replacement than a full one, with the turning point landing around six weeks after surgery.

Poor sleep can persist even after the pain settles

In the same cohort, poor sleep quality was reported by 54 percent of patients before surgery and was still 53 percent at one year, even though night pain had fallen to about 3 percent. Sleep and night pain did not move in lockstep here.

Insomnia tends to peak around week six, then eases

Across a year of follow-up, average insomnia severity rose from before surgery to a high point at six weeks, then fell below the starting level by three months and kept easing through the first year. The roughest sleep window tended to sit in the first weeks, not at the finish.

Among those still in pain at six months, the back of the knee (7.0) and inner side (6.5) score highest; the outer side (3.3) and kneecap (1.5) lowest. Inner-knee ache is the common pattern, not the worrying one.

Two recovery tracks, both normal

By week 8, recovery splits into a fast track (72.4% of people) and a slower one (27.6%). Slower responders still improve — their six-month knee scores trail the fast group, they don't stall.

Kneeling comes back slowly, and for some never fully

Across pooled data, about 34.5 percent of people could kneel within the first year, rising to 36.8 percent by at least a year out and 47.6 percent by three years or more. Kneeling is one of the slowest knee-replacement abilities to return, and for many it stays partial well past the point other movement feels normal.

In a nationwide registry of 64,840 knee replacements, 1.7 percent had a manipulation under anesthesia within the first year because the knee was not bending far enough on its own. So this extra procedure for stiffness is something a small minority of knees go through, not the typical path.

Across pooled data, returning to sport after a knee replacement reads as a curve that climbs with time rather than a single switch. About 18.7 percent were back to sport at 3 months, around 70 percent by 6 months, 84 percent by 12 months, and 87.9 percent by the end of follow-up at a mean of 14 months. A quiet few-months mark is what the averages describe, not a stall.

Most of the bend returns by twelve weeks, then it levels off

In 559 knee replacements tracked over a year, bend went from about 100 degrees at two weeks to 117 degrees at one year, with the largest change happening in the first twelve weeks and motion leveling off by about twenty-six weeks. Straightening barely moved, sitting near 3 degrees short of full at two weeks and 1 degree short at a year.

Meta-analysis puts mean return to work at 12.9 weeks, with a wide honest range (5-42 weeks) driven by job demands. About 90% are back within a year.

Which activities people pick back up, and which fade

In a survey of 784 recreational athletes, low-impact pursuits held roughly steady across surgery while higher-impact ones thinned out. Swimming stayed flat at 62.7 percent before versus 63.6 percent after, and cycling held at 62.3 versus 59.0 percent. Running fell from 84.0 to 48.5 percent and skiing from 72.7 to 45.5 percent. Dropping a high-impact sport while keeping a low-impact one is a common shape here, not a personal failure.
