Weeks 0-2 · the dramatic relief
Most patients wake up with their pre-operative leg pain materially diminished. The Peul cohort reported a fall from 67/100 VAS at baseline to about 28/100 by two weeks — equivalent to going from 6.7/10 to 2.8/10 on a familiar scale. The early-surgery arm of the trial showed about three-quarters of patients reporting clinically meaningful improvement by the two-week mark.
Back pain at the incision site is the new constraint of week one and two. It usually settles to a 2-3/10 level by the end of week two and is well-controlled with paracetamol and short-course anti-inflammatories.
Walking is encouraged from day one in most protocols. Sitting tolerance is typically limited to 20-30 minutes at a stretch for the first two weeks. Most people resume driving around weeks 1-2 if they're off narcotic pain medication.
